EQ-Bold Powder Mixing Issue

I know this might sound stupid but I am stumped. I used the suggested EQ-Bold powder recipe to mix 10g of powder. What I got was a solution that did not mix...a milky solution. Would someone tell me what I did wrong and what I can do to get it correct on the next batch?
