Abstract

The mechanism of action of sildenafil is well documented (Figure 1). An action as a selective inhibitor of the phosphodiesterase (PDE) V iso-enzyme results in potentiation of non-adrenergic non-cholinergic (NANC) neurotransmission. This in turn, under certain situations, produces a pro-erectile response. In cavernosal tissue there is a counter-balancing (flaccidity-inducing) action of the neurotransmitter, noradrenaline, and other locally acting hormones, particularly testosterone can either augment or diminish the NANC activity.
