I started collecting a list of things that you sometimes have to disclose, but only when you meet certain criteria. Any others that I missed?
Structured Settlements: Name of each insurer with annuities in excess of 1% of surplus

Insurers that dont provide collateral, if recoverables exceed 3% of surplus
Source: Notes to Financial Statements/Reinsurance/Unsecured Reinsurance Recoverables
Identify any cessions of >75% of direct premium (could be a fronting carrier). Exemptions:
i. Intercompany cessions
ii. Cessions to a pool/group that underwrites jointly, which is overseen by some state
iii. Gross annual premium is less than 5% of surplus
iv. Cessions to captive insurers, which are regulated in their domiciliary state

If direct premiums written through MGAs/TPAs exceed 5% of surplus, disclose
Name, FEIN
Whether the party holds an exclusive contract
Types of business written
Type of authority granted
Total premium written

Finite reinsurance: Answer if ceded reinsurance resulted in a gain/loss >5% of prior surplus, or ceded premiums/reserves >5% of surplus (and was accounted for as reinsurance, and had one of certain other features)
