In Duncan, on page 255, table 13.3, I don't understand how they get $1105 for the average cost in the intervention year:
.13(5500) + .87(500) = $1150, not $1105.
Even if I use the unrounded percentages starting in the baseline year it doesn't work:
High Risk Percentage in Intervention Year: .15(.33) + .85(.1) = .1345
Then .1345(5500) + (1-.1345)(500) = 1172.50.
